The most carefully built form in the library, because a wrong default here does real harm. Anonymity is the first question, and the name and contact fields appear only if someone chooses to be named — the opposite of the usual pattern, which collects identity up front and offers anonymity as a checkbox at the bottom, by which point the details are already typed.

Frequently asked questions
Is it genuinely anonymous?
If the reporter chooses not to give a name, no name or contact field is ever shown or stored. Turn off any email notification that would reveal a sender, and do not enable IP-level analytics on this form.
Should this replace a safeguarding process?
No. It is a way in, not a policy. The ending tells the reporter that a member of the safeguarding team reads every report the same working day, and to find an adult in the building if they are in danger now — keep that, and make it true.
Can we route it to one inbox?
Yes, with an email notification or a webhook to whatever your safeguarding team already watches.
