Does it collect a real signature?
Yes — a drawn signature field, the same one the waiver templates use. It is stored with the response and exports with it.
Do parents have to answer the medical questions?
Only the ones that apply. Ticking 'no medical needs' skips straight past the detail boxes, so a parent with nothing to report is done in under a minute.
Can I run one slip for several trips?
