A cancellation survey has about fifteen seconds of goodwill and every extra field spends some of it. The reason comes first, on its own, and everything after it is optional. It deliberately does not try to save the customer: a form that answers 'I'm leaving' with an offer is the reason people stop giving reasons, and the reason is the only thing this form is for.

Frequently asked questions

Why no save-offer?

Because it costs you the data. A discount at the moment of cancelling makes the honest answer feel like a negotiating position, and the reason is what you actually needed.

Does it ask where they are going?

Only if they say they are moving to something else — and the copy promises you will not email them about it.

Can it cancel the account too?

No, it records the request. Wire it to your own process with a webhook or a notification.
