A refund request met as a wall of eight fields reads as an obstacle course designed to make people give up — which is exactly what a lot of them are. One question per screen, starting with the order number, reads as being walked through it. Why one question at a time?

Because of who is filling it in. Somebody already annoyed abandons a long form; a single question with a clear end reads as help rather than an obstacle.

Does it process the refund?

No — it captures the request, the reason and what the customer wants to happen. The money moves wherever you already move it.

Can customers attach a photo?

Yes, and it is asked for only when they say the item arrived damaged or faulty.
