Cognito Forms is unusually capable for the price — its Pro plan buys calculations, payments and document merge — but it meters entries at 2,000 a month and caps you at two users. FormSnapp Pro is $19/mo (or $15 billed annually) with unlimited responses — and the free plan is unlimited too. Cognito Forms’s free plan stops at 100 responses a month.
Entries are metered per month and seats are capped per tier — 2 users on Pro, 5 on Team. Storage is capped too, at 1 GB and 10 GB.

Payments are built in
Stripe, PayPal and Square, with the calculated total flowing into the charge. We have no payment field, so an order form here collects the order and takes the money elsewhere.
Document merge and HIPAA
Generate a filled PDF from a submission, and a HIPAA-compliant tier if you need one. Both are real capabilities we do not have.
