A pulse check only works if it is answered every week, and that depends entirely on being finishable in under a minute on a phone between two meetings. Five screens with one thing on each survives that; a page of five fields is the format people mean to do later. Anonymity is stated in the welcome screen rather than asked as a question — a form that asks whether you want to be anonymous has already made you think about who is reading it.

Frequently asked questions
How is this different from an engagement survey?
An engagement survey runs once or twice a year and asks thirty questions. This runs weekly and asks five, because a trend you can see moving is worth more than an annual snapshot.
Is it really anonymous?
It collects no name or email. Keep it that way — turn off any notification that would identify a sender, and share the results as totals and unedited comments.
Should managers see individual answers?
The template's ending promises results go to the whole team on Friday, comments included. That is a commitment worth keeping, and worth changing before you launch if you cannot.
